Identification and Management of Frailty and Complex Care HEA4266

Overview

This level 7 module will facilitate the development of the expert knowledge and skills to identify and manage the frail population across a variety of settings informed by both local and national policy and best practice drivers, such as the NHS long term plan (2019) (1). Learners will develop an ability to employ assessment strategies and tools, evaluating the impact of multidisciplinary interventions and measures within their own practice and the wider health economy. Students will also explore the anatomy and physiology of ageing through the prism of major frailty syndromes developing an ability to critically appraise the assessment strategies and interventions used to enhance ones own practice and service provision aligned to person centred care.

Who is this module for?

Health care professionals who are registered with a professional body for example, NMC, HCPC etc, within the health and social care sector.  The module can be accessed as a standalone module, or form part of a post graduate pathway.

What are the key aims of the module?

  • Develop expert skills and knowledge to identify and manage the frail population within the primary care setting.
  • Encourage exploration of the anatomy and physiology of ageing and perform a critical appraisal of the assessment strategies used in practice.
  • Evaluate the impact of intervention measures within practice and the wider health economy.

How will I be assessed?

You will receive designated academic support as you prepare to submit a 4000 word written assignment in order to meet the assessment requirements for this module.

On successful completion you will:

1. Critically appraise the application of national and local policy and population drivers which aim to optimise the health of the frail population and reduce the risk of inappropriate hospital admissions.
2. Critically analyse the application of frailty assessment tools and evidence based practice guidelines within clinical, home and community environments and apply them appropriately to a diverse range of patients and environs.
3. Demonstrate critical insight into the complexity of the common major frailty syndromes and the associated impact on management and care.
